What Is Melasma? Causes, Triggers, and Treatment Options Explained

Melasma is a common skin condition that causes patches of darker pigmentation, usually on the face. It often appears symmetrically on the cheeks, forehead, upper lip, and chin. Although melasma isn’t medically dangerous, its visible patches can impact how people feel about their appearance, sometimes affecting confidence and overall well-being.

What Is Melasma? 

Melasma is essentially a chronic skin disorder where the cells responsible for skin color (melanocytes) become overactive and produce too much pigment (melanin) in specific areas. This results in symmetrical, flat, brown or gray-brown patches, most commonly on the face.

While it can affect anyone, around 90% of melasma cases occur in women of reproductive age (20–40 years old). 

What Is the Appearance?

Melasma presents as well-defined, flat patches of discoloration that range from light brown to gray-brown or tan. The patches are usually symmetrical, appearing on both sides of the face.

What Are the Causes?

Melasma occurs when the skin produces excess melanin, the pigment that gives skin its colour. Several factors contribute to this overproduction:

  • Hormonal Changes: Pregnancy (sometimes called “mask of pregnancy”), use of oral contraceptives, or hormone replacement therapy can increase melanin production.
  • Sun Exposure: Ultraviolet (UV) light triggers melanin production, worsening pigmentation and causing flare-ups.
  • Genetics: A family history of melasma increases susceptibility, suggesting that certain individuals are more prone to pigment changes.
  • Other Factors: Skin inflammation, certain medications, and environmental factors may also contribute.

What Triggers It?

Even after melasma develops, certain factors can worsen or reactivate pigmentation.

  • Direct sunlight, as well as heat and visible light, can stimulate melanocytes (pigment-producing cells).
  • Aggressive exfoliation, harsh chemicals, or certain cosmetic procedures can trigger post-inflammatory hyperpigmentation (PIH).
  • Shifts in estrogen and progesterone, such as during pregnancy or with certain medications, can prompt melasma to appear or worsen.
  • Some studies suggest that stress and environmental factors may exacerbate melasma.

Who Is Affected?

Besides being more common in women, melasma is more likely to develop in people with medium to darker skin tones. 

What Are the Types?

  • Epidermal melasma: Pigment is in the outer layer of the skin (epidermis). This type tends to respond better to topical treatments and sun protection.
  • Dermal melasma: Pigment is deeper, in the dermis. This type is more challenging to treat and may respond less predictably to creams alone.
  • Mixed melasma: Pigment exists in both epidermal and dermal layers. It often requires a combination of treatments for the best possible improvement.

Treatment Options

Managing melasma typically requires a multi-pronged approach. No single therapy guarantees permanent clearance, and treatment outcomes vary depending on skin type, pigmentation depth, and adherence to sun protection.

Topical therapies, such as hydroquinone, tranexamic acid, azelaic acid, or retinoids, are often the first line of treatment. Laser and light-based therapies can be considered for recalcitrant cases that do not respond sufficiently to creams.

Laser and Light-Based Treatments

  • Low-fluence Q-switched Lasers: Gently break up pigment with low energy. Requires multiple sessions, often combined with topical therapy and strict sun protection. 
  • Non-ablative Fractional Lasers: Create tiny columns of thermal injury to stimulate skin remodelling and disperse pigment. Careful assessment is required for individuals with different skin types and tones.
  • Picosecond Lasers: Use ultra-short pulses to fragment pigment while limiting heat exposure. 
  • Intense Pulsed Light (IPL): Broad-spectrum light that can target superficial pigment. Often reserved for carefully selected patients and used with topical creams and sun protection.

At-Home Management

After any melasma treatment, maintaining results depends on careful post-treatment care.

  • Strict Sun Protection: Daily broad-spectrum SPF 50+ sunscreen, reapplication every two hours outdoors, wide-brimmed hats, and avoiding peak sun hours.
  • Topical Maintenance Therapy: Continue creams such as hydroquinone, tranexamic acid, or retinoids.
  • Gentle Skincare: Avoid harsh scrubs, chemical peels, or aggressive exfoliation that could trigger post-inflammatory hyperpigmentation (PIH).
  • Regular Follow-ups: Clinicians may schedule maintenance sessions or review your treatment plan to monitor for early pigment changes and adjust therapy as needed.
