Non surgical nose job – Nose Thread Lift, Nose Fillers or both?

Non-surgical nose job – more commonly known as nose augmentation (nose threads/ nose fillers) is a commonly requested procedure, especially in the Asia Pacific region, now that aesthetic procedures are becoming more widely accepted. 

The nose thread lift is gaining popularity and it is common these days to have patients attending asking for the treatment. But, one of the most important, and perhaps not asked often enough questions is: Which is better? Nose thread or nose filler? 

Every person is unique and not every nose is the same. Some noses are more suitable for thread lifts and some will benefit more from fillers. To best correct or enhance the appearance of a nose, we first have to understand what each treatment excels at: 

Nose threads are more effective in creating definition and is also more effective at creating a sharper nose tip than filers are. These threads are designed with small anchoring points (also known as cogs/ barbs) to hook onto tissues within the nose to create a lifting effect. 

Fillers, on the other hand, provide volume to the nose. However, the choice of fillers is important as certain fillers can shift or spread over time, causing a broader looking nose instead.


But, how do we know which is better? 

To answer this, we need to first look at which are the most common presentations when it comes to people requesting for non-surgical nose jobs. 

The common presentations can be summarized into: 

  1. Low nose bridge
  2. Bulbous nose tip/ round nose
  3. Dorsal hump/ crooked nose 

I want a higher nose
In patients with a low/ flat nose bridge, nose fillers are generally the better option as it can give a larger amount of augmentation and volume. To achieve a large amount of augmentation, a large number of threads are generally needed, which increases both the cost, as well as the risk of complications.

Some patients, on the other hand, may have some height already on their bridge, and may just want a better-contoured nose. These patients may be more suitable candidates for nose thread lift. 


I want a more defined nose/ my nose is too round
For patients with a bulbous nose tip/ round nose, this usually requires an in-depth assessment of the nose. A round appearing nose may be contributed by the appearance of a flat bridge and a bulbous nose tip. This can usually be addressed by a combination of methods. 

In patients with a flat bridge as well as a bulbous tip, improving the height of the bridge can actually give a more triangular appearance to the nose and elongate the appearance of the nose. Threads, on the other hand, are more effective at creating a sharper nose tip and can make the nose appear smaller and sharper. 


Correcting a crooked nose/ I want a straighter nose
The last issue is that of a crooked nose. This is typically caused by a hump at the keystone area of the nose. This can be corrected with either fillers or threads depending on the area of the bump/ depression, individual condition and desired effect.

Can I use a combination of both fillers and threads?
This is a very commonly asked question. It is possible and even beneficial in certain cases that a combination of both fillers and threads are used. This is usually done to either achieve a more dramatic effect or to address a combination of issues to enhance the appearance of the nose. When used together, fillers are typically to attain the desired height for the bridge, before threads are inserted to contour, define and lift the nose tip. Results also typically last longer if a combination method is used. 

How long does the effect of the treatment last?
Nose threads are made with PDO/ PLLA which are the same materials used to make absorbable surgical sutures. It is naturally broken down and metabolized by the body.

The effects typically last 1-2 years depending on the type of material, lifestyle and individual variations. 

Hyaluronic acid-based fillers typically last 3-12 months depending on lifestyle and body absorption rate, whilst non-hyaluronic acid fillers last longer (18-24 months) 

What if I don’t like the effect of the threads/ fillers?
Nose threads and hyaluronic acid-based fillers are non-permanent. They will typically be absorbed within 9-12 months naturally by the body. Hyaluronic acid-based fillers can be dissolved with an antidote as well.
This is also why it is important to discuss with your doctor the desired outcome in a consult before making an informed decision about which procedure to undergo. 

Are nose thread lift/ nose fillers safe?
The procedures are generally safe. Nose fillers have a very rare complication of blindness but nose thread lift has mostly self-limiting risks such as bruising, thread complications and infection which are rare.
